Information is vital to your success! Make sure you answer each question on your forms completely and make sure we get copies of any lab tests obtained during the last six months.
Step One: Fill out the Patient Intake Form
This form is important because your health history reveals a lot about your lifestyle choices and how they impact your health. Take the time to consider each question carefully but don't go crazy with unnecessary details.
Step Two: Fill Out Body Chemistry Balance Evaluation
This form is important for us to determine the current health and state of balance of your microbiome. Your microbiome is vital to your health. It is estimated that 70% of your immune response is the responsibility of the microbiome. Not only that, but your microbiome is responsible for the manufacture of many vitamins and nutrients required by your body to stay healthy, as well as for the health of the lining of your gut. If this lining is compromised, your body cannot absorb necessary nutrients or eliminate toxins.
Since your microbiome involves 10X as many cells as cells that carry your DNA, you cannot be any healthier than your microbiome. The health of the microbiome is compromised by refined foods, gluten containing foods, water treated with chloramine (most water supplies add it) and fluoride or water containing dangerous chemicals, and stress!
